- 1 MFT or 1 MSW intern
- 20-24 hours per week, Monday through Friday
- Required day: Wednesdays from 9am-4pm. Remaining days and times TBD
- Placements available for 9-12 months beginning in September
Program Description
Older Adult Mental Health provides a continuum of care to seniors throughout Contra Costa County with teams that provide outreach, engagement, and on-going services to the most imperiled seniors in the county.
Our Intensive Care Management Program consists of three multidisciplinary teams who provide field-based services to older adults (60+) diagnosed with a chronic and severe mental illness in their homes, board and care homes, shelters, etc. The Intern will be working with a multi-disciplinary team, which includes a Geropsychiatrist, Nurse, Mental Health Clinicians, and a Community Support Worker. The intern will gain experience in several areas, including providing case management services to older adults, collaborating with family members and other service providers, and providing crisis intervention/risk assessment as needed. In addition, there can be the opportunity to complete intakes, and provide supportive therapy.
The IMPACT Program provides short-term Cognitive Behavioral and Problem-Solving therapy to older adults (55+), who often have co-occurring medical diagnoses, such as diabetes, cardio-vascular disease, and/ or chronic pain as well as moderate to severe depression, anxiety or Post-Traumatic Stress Disorder.
This is a field-based position, which will be primarily located in Central County, but may require travel to East and/or West County regions. A stipend will be provided by Contra Costa County.
