Contra Costa Health was forced to put Service Excellence Awards on hold during the pandemic, but our staff continued to go the extra mile and care for our communities. We are excited to recognize the hard work of our amazing team at Contra Costa Health.

For 10 years of dedicated service to first time parents in the Contra Costa community and excelling in supporting the families they serve. For improving maternal health, infant health and development, and increasing family economic self-sufficiency through the maternal child home visitation Nurse-Family Partnership (NFP) program. For the high-quality nursing intervention provided to 575 families with the outcome of 92% full-term newborns, 99% receiving breastmilk, and 97% up to date on immunizations at 24 months of age. For being recognized nationally as a leader in the NFP, including the development of the Goal Mama App, and across multiple National Innovations Advisory committees. For adhering closely to model fidelity to provide high quality nursing care as well as being highly accomplished in reflective practice and supporting nurse wellness resulting in minimal nurse turnover over the last decade, including their dual service to the Community of Contra Costa and NFP families during the COVID-19 pandemic.
